FY09 Outcomes

During the 2010 Fiscal Year (July 1, 2009-June 30, 2010), CHIP staff conducted 7,105 home visits with 1,104 children from 773 families.

 

FAMILY STRENGTHENING

  • Of parents who were unemployed and seeking employment at enrollment, 39% became employed after two years in the program.
  • Of parents with less than a high school education or GED at enrollment, 28% obtained their GED or high school diploma after 2 years in the program.
  • Of parents who had obtained a high school diploma or GED by their CHIP enrollment, 28% improved their education status through vocational training, college, or other institutions of higher education.
  • 100% of enrolled children enrolled who turned 5 by September 30 had the required documentation to enter kindergarten.
  • At age 5, 84% of enrolled children scored at or above the standard developmental milestones in each area as measured by the Ages and Stages Questionnaire.

 

HEALTH CARE COORDINATION

  • 98% of enrolled children reported having a medical home.
  • 62% of enrolled children reported having a dental home.
  • 100% of newly enrolled children were assessed for developmental delays within 3 months of enrollment.
  • 28 children were determined to be at-risk for developmental delays and were referred for intervention services.
  • 90% of families with enrolled children age birth to five received nutritional counseling through WIC. 75 families declined WIC services.
  • 46% of enrolled children had an initial dental exam by age three.
  • 68% of enrolled children with a propensity for using the emergency room for primary care, showed a reduction in ER visits at annual recertification.
  • 92% of enrolled children turning two during the timeframe received the recommended number of immunizations by the age of two.
  • 94% of enrolled children completed their immunization series by age six.
  • 90% of enrolled children were up-to-date on recommended well-baby visits.
  • 48% of enrolled children age three and over received regular dental care.

 

PEDIATRIC ASTHMA PROJECT

CHIP’s Health Care Advisory Committee is developing new outcomes for CHIP’s Asthma Project.  This set of new data will be posted by year end.

 

BEGIN WITH A GRIN DENTAL VARNISH PROGRAM

  • 90% of children who turned 37 months during the timeframe received at least one dental varnish application before turning 37 months.
  • CHIP nurses applied 578 dental varnish treatments to 439 children during the timeframe.
  • 99% of families with children participating in the Begin With A Grin program received in-home dental education using the Virginia Department of Health’s Bright Smiles for Babies curriculum.

 

PREGNANT MOMS PROGRAM

  • 87% of enrolled women were 90% up-to-date on recommended prenatal visits.
  • 62% of enrolled women who smoked, quit smoking during   pregnancy.
  • 45% of enrolled women breastfed their babies for the first two months postpartum.
  • 96% of enrolled women were enrolled in WIC for supplemental nutrition and counseling during pregnancy.
  • 67% of enrolled teen women who lacked a High School Diploma or GED, continued their education after the birth of their baby.

 

MENTAL HEALTH

  • 100% of children who enrolled in CHIP Mental Health Case Management received a complete psycho-social assessment through Blue Ridge Behavioral Healthcare.
  • 73% of enrolled children age 5 and under, improved their social-emotional health from the previous year as measured by the Ages & Stages Questionnaire: Social-Emotional Assessment.
  • 42% of enrolled children ages 4-7 realized a 10 point reduction in at least two areas of functional impairment, as measured by the PECFAS (Preschool and Early Childhood Functional Assessment Scale), compared to the previous year’s score.
  • 75% of enrolled children who received a recommendation for outpatient counseling complied with and received the service.
  • 100% of enrolled children have a service plan developed based on their individual needs.
