CHIP of Roanoke Valley is seeking a compassionate Registered Nurse to join our home visiting team. Our Community Health Nurses work directly with pregnant caregivers and families with young children, providing individualized health education, support, care coordination, and connections to resources that help children and families thrive.

This is nursing outside of the traditional hospital or clinic setting. You’ll have the opportunity to get to know the families you serve, understand the challenges they face, celebrate their progress, and make a meaningful impact on their health and well-being.

WHAT YOU’LL DO
As a Community Health Nurse, you’ll manage your own caseload and work with families in their homes, in the community, and as part of CHIP’s multidisciplinary team.
Your work will include:
• Providing regular home visits to pregnant caregivers and families with children from birth through kindergarten entry
• Providing prenatal, postpartum, parenting, injury prevention, and health promotion education
• Helping families understand their child’s health and healthcare needs
• Conducting health and family assessments to identify strengths, needs, and potential concerns
• Providing asthma education and case management
• Reviewing medical information and helping families understand recommendations from healthcare providers
• Coordinating care and connecting families with healthcare providers and community resources
• Building trusting relationships with families from a wide variety of backgrounds
• Collaborating with other CHIP team members to help families identify and achieve their goals

COMPENSATION & BENEFITS
Starting Pay Range from $57,590-$65,077, based on qualifications and experience.

CHIP offers a comprehensive benefits package for eligible employees, including:

• Paid holidays based on the Virginia State Holiday calendar

WHO WE’RE LOOKING FOR
• Current, valid Registered Nurse (RN) license in the Commonwealth of Virginia
• At least two years of related nursing experience
• Pediatric experience preferred
• Strong communication, organization, and relationship-building skills
• Ability to work independently while also contributing to a collaborative team
• Commitment to providing respectful, culturally responsive care to families in the community
