CHIP of Roanoke Valley is seeking a compassionate Family Case Manager who can build trusting relationships, empower families, and promote positive parenting skills. As part of our home visiting program serving pregnant individuals and families with children from birth through kindergarten entry, you will build strong relationships, identify and support family strengths, and help create lasting positive outcomes.

Do you have a desire to assist families in our community? Would you like to use your skills to help families achieve self-sufficiency? CHIP of the Roanoke Valley is a home visiting program caring for pregnant women and families with children from birth to kindergarten entry. We are searching for a candidate who will create meaningful relationships with our clients to build on their strengths and create a difference in their lives. Job duties include: home visits, asthma case management, prenatal/parenting education, conducting assessments on health, nutrition, psychological, environmental and economic demographics of clients, working with medical reports, providing education on potential health problems and understanding of specific health care needs and providing resources to families. If you are interested in being a part of a culturally sensitive team that will help a family create meaningful change in their lives, then we would love to speak with you!
Candidates must have a valid RN license for the Commonwealth of VA and have 2 years of related work experience. Experience in pediatrics is preferred.
