The proposed Early Intervention Collaborative will target low-income & high-risk Pregnant Moms and Moms with children 0-36 months of age through a comprehensive, holistic approach to case management and resources provided by partnering non-profit and other community agencies. This includes prenatal and postpartum case management, safety assessments, shaken baby syndrome education, car seats, child development education for new mothers etc. Each family will be linked to medical & dental homes, have access to mental health support & counseling, and be provided with family strengthening resources that ensure the greatest chance for success for children and families receiving interventions at the earliest stages possible. Childcare and literacy development will remain a program-focused support area, while other resources such as food (LEAP) and after school, summer programs will be made available and referred to Youth and Adult collaboratives through UWRV to better serve the needs of older siblings in need of these resources. Of note, CHIP would continue the (funding dependent) use of providers for dental and medical home establishment, and care for the adult caretakers of these families (Bradley Free Clinic, NH, etc) in order to continue a multi-generational approach (begun 5 years ago through the ELC).
The project will capture a finite number of families and measure their performance and achievement of stated metrics over a three-year period. Resource referrals to youth and adult collaboratives will be measured and tracked as outputs throughout the period. At the conclusion of the 3-year period, CHIP will be able to provide data on all families and measure how the earliest intervention approach to case management had an effect on these families.
Service goal: 1st year – 100+ families (250+ enrolled children and adults) (30-40 pregnant moms + 0-12 cohort of 80+)
The first 2,000 days of life lay the foundation for a child’s future health, academic and workplace success. During the First 2000 Days, brain architecture is forming, creating the foundation for all future learning. Child development is a dynamic, interactive process that is not predetermined. It occurs in the context of relationships, lived experiences and surrounding environments. For optimal development and a strong foundation, children need good health, secure families and high-quality early learning experiences. Research shows home-visiting programs, like, CHIP, are the most effective tool to ensuring all children have everything they need to get most out of their first 2,000 days of life. CHIP’s model provides evidence-based home-visiting for vulnerable children living in poverty under the age of 5 and their families.
By focusing on the five universal family strengths identified in the Strengthening Families Protective Factors Framework*, community leaders and service providers can better engage, support, and partner with parents in order to achieve the best outcomes for children during early childhood.
1. Enrollment Cohort
Referring partners include CHIP of the Roanoke Valley, the McKinney-Vento Program serving RCPS, Department of Social Services, Roanoke Health Department (Baby Care), and self- and clinician referrals.
2. Address Social Determinants Of Health
Families are linked to medical & dental homes, supported in engaging in preventative healthcare, provided with mental healthcare, supported in locating stabile housing, connected to development and training programs for the unemployed, and provided with family strengthening resources.
3. Perinatal Resources
Through resources and education specific to the perinatal period, this collaborative focus area aims to improve the health of women, infants and children through improved access to and enhanced utilization of perinatal and prenatal care and related services.
4. Early Childhood Resources
Following the >36 months service period, families remain enrolled in CHIP’S comprehensive program until the qualifying enrolled child enters kindergarten.
5. Referral stage
The collaborative will utilize Unite Us to streamline the process of connecting discharged families to community organizations, thereby bridging access to holistic, whole-person care.
