Health Care Coordination
Child Health Investment Partnership connects caretakers and their children at enrollment with doctors, specialists, pediatric dentists and a counselor if needed to make sure appointments are scheduled and kept, enhancing the health of each child. Child Health Investment Partnership reviews medical records and coordinates care with the child’s doctor and dentist. Transportation is provided to these appointments to enhance access to care.
Health Care Coordination is specific to the needs of each family which may begin with Pregnancy Education for the mother or children discharged from the Neonatal Intensive Care Unit. Registered nurses may provide asthma case management, oral health education and fluoride varnishes, nutrition education and smoking cessation strategies. Child Health Investment Partnership nurses also ensure that children receive and complete recommended immunizations, well child visits, lead screenings and developmental interventions by kindergarten entry.
