Frequently asked questions

Who can refer a family?

Anyone! Community health workers, friends, nurses, physicians, state agencies, etc. You can even refer yourself.

Absolutely, most of our referrals are self-referrals.

  • Anyone living in Roanoke City, Roanoke County, Salem, Bedford County, Botetourt County, or Craig County
  • Any (parent, guardians, grandparents, etc.) that have one or more children under the age of 4, or are expecting a child
  • A child or family that has Medicaid or is eligible for Medicaid, unless ineligible due to immigration status.
  • A guardian who is available at least one day per month for a scheduled home visit Monday-Friday between 8 am and 5 pm

No! Our program is completely free to enrolled families.

Yes! We have a dedicated Spanish-speaking team. CHIP can also serve any language with the assistance of translation services.

Once we receive the referral, our intake specialist will reach out to do an eligibility check. If you qualify, then a Family Case Manager will schedule your first home visit.
